Job Description

Meadows at Brookmeade located in Rhinebeck, NY, is seeking a compassionate and motivated Social Work Assistant II to join our dedicated team. This position will report to the Director of Social Work. You will assist in admits, discharges, care planning, conducting assessments, and coordinate services to enhance the well-being and quality of life of our residents.

Responsibilities:

1. Review referrals for admission
2. Review insurance and verify benefits
3. Meet with families and conduct facility tours
4. Build relationships with hospitals, social workers, and discharge planners
5. Maintain an accurate facility census and knowledge of available beds
6. Help residents and family members to complete paperwork as needed
7. Assist Marketing and Admissions Director with tasks as needed
8. Assists with discharge, admissions planning and other case management duties
9. Clerical duties

Qualifications:

1. Bachelor's degree in Social Work, Psychology, Human Services or other related field
2. Previous experience in long-term care, healthcare preferred
3. Strong communication and interpersonal skills
4. Ability to work collaboratively in a team environment
5. Ability to prioritize and execute quickly in a fast-paced environment
6. Proficient in documentation and record-keeping practices
7. Compassionate and customer-focused approach
